This study allows 3D or IMRT planning. Credentialing is required if IMRT is used.

IMRT credentialing:

• Complete and submit the IMRT Facility Questionnaire.

• Irradiate the IROC Houston's IMRT H&N phantom.¹  Please fill in the phantom request form online.

• If an institution is using IMRT with gating or tracking, the institution must irradiate IROC Houston’s thorax-lung phantom with motion platform. Please fill in the phantom request form online.

Note:

¹ If your institution previously completed a pelvic phantom irradiation, that irradiation may satisfy the credentialing requirements for this protocol. Institutions that have previously completed a successful irradiation of the IMRT H&N phantom may not need to repeat the phantom irradiation for credentialing for this protocol. Institutions that are credentialed to participate in other IMRT protocols should fill in the Credentialing Status Inquiry Form.
