This trial will utilize TRIAD for dosimetry digital treatment data submission. TRIAD is the American College of Radiology’s (ACR) image exchange application. See here for information on installing TRIAD.

Institutions who wish to credential for this protocol or have a question about your status for this protocol, Please fill out the Credentialing Status Inquiry Form

The credentialing steps for TBI must be met within 6 months following the TBI treatment of the first patient enrolled on this study at each participating site. Photons, electrons, or proton therapy are allowed on this protocol.  

In order to complete the TBI credentialing  process, an institution must:

•  Complete or update the Facility Questionnaire.

•  Successfully irradiate the IROC Houston's TBI phantom. Please fill in the phantom request form online.

In order to complete the IMRT credentialing process, an institution must:

•  Complete or update the IMRT Facility Questionnaire.

•  Successfully irradiate the IROC Houston's IMRT H&N phantom. Please fill in the phantom request form online.

Note: Institutions that have prior IMRT credentialing may not need to re-irradiate the H&N phantom.

In order to complete the Proton Therapycredentialing process, an institution must:

• Complete or update the proton Facility Questionnaire.

• Irradiate the IROC Houston's proton H&N phantom. Please fill in the phantom request form.
