CALGB 30609 Requirements
This study requires credentialing before patients can be entered onto this study. The credentialing requirements are different depending on whether you are using a 3DCRT or IMRT technique. You can be credentialed for either technique or for both depending on your physician's treatment preference.
In order to complete the 3D credentialing process, choose below your treatment planning system (TPS) / algorithm to see the requirements for credentialing:
Helax / collapsed cone
Eclipse / AAA
XiO /superposition or fast superposition
Pinnacle / collapsed cone or adaptive convolution superposition
Tomotherapy / convolution super position
If your TPS is not listed above please contact the IROC Houston at 713-745-8989 to determine what is needed to complete the credentialing process.
In order to complete the IMRT credentialing process, the following items must be completed:
Complete and fax/e-mail IMRT/IGRT Facility Questionnaires to RTOG at 215-940-8817 or rtog-facquest@phila.acr.org.
Irradiate the IROC Houston's Lung phantom using an IMRT technique (Institutions already credentialed to participate in other RTOG IMRT protocols may not have to irradiate a phantom, see the Note below). Please fill in the request form online.
Note:
Institutions that are credentialed to participate in other RTOG IMRT protocols need to fill in the credentialing status inquiry form to verify their ability to enter patients onto this protocol.
Submission of the lung benchmark case electronically to the ATC and hardcopies to the IROC Houston (c/o RTOG 0617 Credentialing, 8060 EL RIO St. , Houston, TX 77054) might be required depending on the results of the credentialing status inquiry form.
Irradiation of the lung phantom might be required depending on the results of the credentialing status inquiry form.
