In order to complete the 3D-CRT credentialing process, the following items must be addressed:

• Complete and fax/e-mail the 3D-CRT Facility Questionnaire to RTOG at 215-940-8815 or rtog-facquest@phila.acr.org.

• Submit a dry run test electronically to the ATC.

Note:

• Submission of the lung benchmark case has been removed as a requirement for credentialing for these treatment planning systems/algorithms.
