GOG 0263 Requirements
This protocol requires each institution to submit electronic patient data to GOG using TRIAD. For instructions on TRIAD, click here. Please note, with CTSU integration with TRIAD, IRB approval is required in order to submit credentialing data.
This study requires credentialing before patients can be entered onto this study. The credentialing requirements are different depending on whether a 3DCRT or IMRT technique will be used.
In order to complete the credentialing process the following items must be completed:
• Each Radiation Oncologist must complete and submit online a Knowledge Assessment to the IROC Houston.
• A CTSU IRB/Regulatory Approval transmittal sheet should be submitted along with the CTSU IRB Certification Form or its equivalent.
For 3DCRT, if the above 3 requirements ( Knowledge Assessment and CTSU IRB/Regulatory Approval and CTSU IRB Certification Form) have been completed the radiation oncologist may treat using a 3DCRT technique.
In order to complete the IMRT credentialing process the following items must be completed:
• Complete and submit an IMRT Facility Questionnaire to IROC Houston.
• Irradiate the IROC Houston's IMRT H&N.1 Please fill in the phantom request form online. The "IMRT Benchmark" does not satisfy the credentialing requirements for this protocol.
Note:
1 If your institution previously completed a pelvic phantom irradiation, that irradiation may satisfy the credentialing requirements for this protocol. Institutions that are credentialed to participate in other IMRT protocols requiring a phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol. Institutions that are credentialed to participate in other IMRT protocols should fill in the Credentialing Status Inquiry Form.
