The 3D-CRT credentialing process requires that the following items be completed:

• All participants are asked to complete and fax/e-mail the Facility Questionnaires to RTOG at 215-940-8817 or rtog-facquest@phila.acr.org. Questions about the questionnaires can be directed to Tammy McGlade at 215-574-3219.

• Submit an actual patient treatment plan digitally to the ATC.

•  Submission of the lung benchmark case electronically to the ATC and by hard copy to the IROC Houston (Attn: RTOG lung benchmark, 8060 EL RIO St., Houston, TX 77054) is required. Please include a cover page with Institution name, contact information and the number of the RTOG protocol for which the institution wishes to be credentialed.

Note:

•  Institutions that are credentialed to participate in other RTOG protocols for this disease site must fill in the credentialing status inquiry form to verify their ability to enter patients onto this protocol.
