In order to complete the 3D-CRT credentialing process, the following items must be addressed:

• Complete and fax/e-mail the 3D-CRT Facility Questionnaire to RTOG at 215-940-8815 or rtog-facquest@phila.acr.org.

• Submit the lung benchmark case (see note below) electronically to the ATC and hardcopies to the IROC Houston at the following address:

Note:
Institutions that have passed the IROC Houston lung phantom irradiation test do not need to submit the lung benchmark case.
