Please fill out the credentialing status inquiry form to let us know that you would like to be credentialed for this protocol.

This trial will utilize TRIAD for dosimetry digital treatment data submission. TRIAD is the American College of Radiology’s (ACR) image exchange application and it is used by the NRG. See here for information on installing TRIAD.

Institutions need to credential for only the most complex modality they intend to use. The increasing complexity level for treatment modalities is 3DCRT, IMRT and VMAT.

In order to complete the SBRT and IMRT credentialing process, the following items must be completed:

• All participants are asked to complete the Facility Questionnaire.

•  Successfully complete Boney and Soft Tissue IGRT credentialing. Please also complete the IGRT questionnaire.

• Irradiate the IROC Houston's Lung phantom and a phantom treated using IMRT. Please fill in the request form online.

• All participants are asked to submit the first patient for each disease site randomized to ARM 2 (SBRT) as a pre-treatment review (Section 12.1). Please allow 3 business days (once all data is submitted correctly) for this review to be completed. An email will be sent to all contacts listed on the DDSI form informing them of the outcome of the pre-treatment review..
