In order to start the credentialing process the following must be completed prior to enrolling patients on study:

• Each Radiation Oncologist must Complete and submit a Knowledge Assessment Form to the IROC Houston.

Once the above is complete then following 2 items must be done, per institution, in order to complete the credentialing process:

• Submit intracavitary implant data on your first 2 patients (both LDR and HDR) directly to the IROC Houston.

• Complete and submit a Protocol Compliance Form for each patient to the IROC Houston.
