Please fill out the credentialing status inquiry form to let us know that you would like to be credentialed for this protocol.

This protocol requires each institution to submit electronic patient data to RTOG using TRIAD. For instructions on TRIAD, click here. Please note, if you do not yet have IRB approval but wish to submit credentialing data, please contact Triad-Support@acr.org.

In order to complete the IGRT credentialing process, the institution must complete the additional requirements detailed on the RTOG webpage

In order to complete the credentialing process the following items must be completed:

• All participants are asked to update or complete a new IROC Facility Questionnaires

Dry Run/Benchmark Submission

• 1106 Datasheet (A4)

• Dry Run/Benchmark Case Download Instructions

• Dry Run/Benchmark Mock Patient Summary and FAQ

• DICOM Files for Download (single .ZIP file)

• Submit DDSI Form

• Irradiate the IROC Houston's lung phantom.  Please fill in the request form online.

Note:

Institutions that were previously credentialed to participate in another RTOG lung protocol or have a question about your status for this protocol, please fill out the credentialing status inquiry form.
