This study requires credentialing. The credentialing requirements are different depending on whether you are using a 3DCRT or IMRT technique. You can be credentialed for either 3DCRT or IMRT or both depending on your physician’s preference.

In order to complete the 3D credentialing process, an institution must:

•  Complete the 3D benchmark. Questions about the 3D benchmark can be directed to IROC Houston at 713-745-8989.

In order to complete the IMRT credentialing process, an institution must:

• Complete and submit the IMRT Facility Questionnaires .

•  Irradiate the IROC Houston's IMRT H&N phantom.¹  Please fill in the request form online.

OR

•  Complete and submit the IMRT Benchmark to IROC Houston

Note:

1. Institutions that have previously completed a successful irradiation of the IMRT H&N phantom may not need to repeat the phantom irradiation for credentialing for this protocol. Institutions that are credentialed to participate in other IMRT protocols should fill in the Credentialing Status Inquiry Form.
