A proton facility must complete baseline phantom irradiations for each proton delivery modality they plan to use for clinical trials (e.g. scattered, uniform scanning, modulated pencil beam scanning). The required phantoms are:

 

•    Proton Prostate

 

•    Proton Thorax

   

•    Proton H&N

   

Phantoms can be requested using the Phantom Request Form. Please note that specific cooperative group protocols may have additional phantom irradiation requirements.The list of required phantoms was updated 7/1/2019, so institutions that were approved prior to that date may have completed different baseline phantoms.
