This trial will utilize TRIAD for dosimetry digital treatment data submission. TRIAD is the American College of Radiology’s (ACR) image exchange application and it is used by the RTOG. See here for information on installing TRIAD.

For this study, when credentialing for 3D and IMRT or protons, IGRT is required.

In order to complete the 3D and IMRT credentialing process the following items must be completed:

•  All participants are asked to complete the Facility Questionnaires.

• All participants must complete IGRT credentialing for bony and soft tissue alignment. Please also complete the IGRT questionnaire.

•  Irradiate the IROC Houston's IMRT H&N phantom¹. Please fill in the phantom request form online.

•  Irradiate the IROC Houston's Lung phantom¹. Please fill in the phantom request form online.

In order to complete the Proton credentialing process, the following items must be completed:

• All participants are asked to complete the  Proton Facility Questionnaire .

• All participants must have completed baseline approval for proton therapy.

•  Irradiate the IROC Houston's proton H&N phantom². Please fill in the phantom request form online.

•  Irradiate the IROC Houston's proton Lung phantom². Please fill in the phantom request form online.

• All participants must complete IGRT credentialing for bony and soft tissue alignment. Please also complete the .

Note:

1. Institutions that are credentialed to participate in other IMRT protocols requiring an IMRT H&N phantom and/or lung phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol.
2. Institutions that are credentialed to participate in other Proton protocols requiring a proton H&N phantom and/or proton lung phantom irradiation need not repeat the phantom irradiation for credentialing for this protocol.
