This study requires credentialing if you plan to use 3DCRT, IMRT, or proton therapy. You can be credentialed for any or all techniques, depending on your physician’s preference.

In order to complete the IMRT credentialing process, an institution must:

• Complete or update the IMRT Facility Questionnaire.

• Successfully irradiate the IROC Houston's thorax phantom. Please fill out the phantom request form online.

In order to complete the Proton Therapy credentialing process, an institution must:

• Complete baseline approval for proton therapy.

• Complete or update the proton Facility Questionnaire.

• Successfully irradiate IROC Houston’s PROTON thorax Phantom. Please fill out the phantom request form online.
