In order to complete the credentialing process the following 2 items must be completed:

• Complete and submit the Prostate Brachytherapy QA Knowledge Assessment Form.

• Complete and submit the Prostate Brachytherapy Facility Questionnaire.

If there is more than one radiation oncologist who wishes to be credentialed, please refer to the memo below.

If you have already completed the above for any of the following protocols, RTOG 98-05, RTOG P-0019, RTOG P-0232 or ACOSOG Z-0070, your institution is credentialed.
