In order to complete the IMRT credentialing process the following 3 items must be completed:

• Complete and submit a Facility Questionnaire to the Advanced Technology Consortium ( ATC )

•  Submit an actual patient treatment plan digitally to the ATC .

• Irradiate the IROC Houston's IMRT H&N phantom.  Please fill in the request form online.

Note:

If you are credentialed for either of the following protocols: H-0022 or H-0225, then no further credentialing is necessary for this protocol.
