Institutions that were previously credentialed to participate in another protocol or have a question about your status for this protocol, please fill out the Credentialing Status Inquiry Form.

This study requires credentialing if you plan to use IMRT, SBRT or proton therapy. You can be credentialed for any or all techniques, depending on your physician’s preference. There is no specific credentialing required for use of 3D conformal therapy.

In order to complete the IMRT credentialing process, an institution must:

• Complete and submit the IMRT Facility Questionnaire.

• Successfully irradiate the IROC Houston's IMRT H&N phantom.1 Please fill out the phantom request form online.

• In addition, if patients are treated with IMRT together with gating or tracking methods, successfully irradiate the IROC Houston's thorax phantom. Please fill out the phantom request form online.

Note:

• Institutions that have prior IMRT credentialing with either the IMRT H&N phantom or the IMRT Benchmark do not need to irradiate the H&N phantom.

In order to complete the SBRT credentialing process (which is encouraged for bone metastases <5 cm), an institution must:

• Complete and submit the IMRT Facility Questionnaire.

• Successfully irradiate IROC Houston’s Spine Phantom. Please fill out the phantom request form online.

• Submit the Motion Management Questionnaire.

In order to complete the Proton Therapy credentialing process, an institution must:

• Complete baseline approval for proton therapy.

• Complete or update the proton Facility Questionnaire.
