This trial will utilize TRIAD for dosimetry digital treatment data submission. TRIAD is the American College of Radiology’s (ACR) image exchange application. See here for information on installing TRIAD.

3DCRT, IMRT, or proton therapy are allowed on this protocol. Credentialing is required for IMRT or proton therapy.

In order to complete the IMRT credentialing process, an institution must:

•  Complete or update the IMRT Facility Questionnaire.

•  Successfully irradiate the IROC Houston's IMRT H&N phantom. Please fill in the phantom request form online.

•  In addition, if patients are treated with IMRT together with gating or tracking methods, successfully irradiate the IROC Houston's thorax phantom. Please fill out the phantom request form online.

Note: Institutions that have prior IMRT credentialing may not need to re-irradiate the H&N phantom.

In order to complete the Proton Therapycredentialing process, an institution must:

• Complete baseline approval for proton therapy.

•  Complete or update the proton Facility Questionnaire.

• Irradiate the IROC Houston's proton H&N phantom. Please fill in the phantom request form.

•  In addition, if patients are treated with proton therapy together with gating or tracking methods, successfully irradiate the IROC Houston's PROTON thorax phantom. Please fill out the phantom request form online.
