This study allows 3D, IMRT or proton therapy. Credentialing is required if IMRT or proton therapy are used. 

IMRT credentialing:

• Complete and submit the the IMRT Facility Questionnaire.

• Irradiate the IROC Houston's IMRT H&N. Please fill in the phantom request form online.

Proton credentialing:

• Complete and submit the the Proton Facility Questionnaire.

• Irradiate the baseline proton phantoms required for approval of your proton beam delivery modality. Details can be found here.To request the phantoms, Please fill in the phantom phantom request form online.

Note: Institutions that have previously completed a successful irradiation of the IMRT H&N phantom may not need to repeat the phantom irradiation for credentialing for this protocol. Institutions that are credentialed to participate in other IMRT protocols should fill in the Credentialing Status Inquiry Form.
